It doesn’t matter if you have been recently diagnosed with Diabetes or have had the condition for years, understanding and maintaining your health goals for your condition is important.
Burwood Health now offers appointments with our Diabetes nurse to discuss your condition, either for your annual review or ongoing management.
ANNUAL DIABETES REVIEW
Your annual Diabetes Review is a 1 hour appointment, during this time you will spend 30-minutes with our Diabetes Nurse and 30-minutes with the practice’s Health Coach.
You may also be eligible to have a Care Plan created with our team, speak to the Diabetes Nurse to find out more.
An annual Diabetes review is an important part of your Diabetic Care Management and is a great way to touch base with your clinical team about your health condition and any changes to your lifestyle.
An annual check includes:
- Yearly foot checks
- 2-yearly Retinopathy screening
- 6-monthly blood tests (including HbA1c)
- 1-yearly blood tests (including Electrolytes, Lipids and more)
- Blood pressure
- Weight
6-MONTH DIABETES CHECK
After your 6-monthly HbA1c test, you may be contacted by our team for a follow up Diabetes appointment.
This is a 15-minute appointment with our Diabetes Nurse, which will give you an opportunity to discuss changes to your recent results and create a plan for the next 6-months.
You are also welcome to book a 30-minute appointment with the practice’s Health Coach at the same time.
